Rose: The day before we're all supposed to be rescued, the person that we count on the most suddenly comes down with a life-threatening condition, and you're chalking it up to bad luck?
Bernard: Well, what are you saying, that—that Jack did something to offend the gods? People get sick, Rose.
Rose: Not here. Here, they get better.

At the beach, Jack suddenly collapses from the pain in his abdomen. Juliet deduces that he has appendicitis, and plans to operate on it, with Bernard providing anaesthetic. Jack insists on being awake for the surgery, with Kate holding up a mirror so that he can direct it. While retrieving medical supplies from the Staff, Jin realizes that Charlotte speaks Korean, and demands that she take Sun off the Island. Juliet has Bernard knock Jack out when the surgery starts going badly, and finishes the job herself. Afterwards, she tells Kate that she is aware of Jack's feelings for her, and ends her relationship with him.

In the jungle, Sawyer, Claire and Miles stumble across the graves of Danielle and Karl. They are found by Lapidus, who warns them to hide before Keamy comes back. Lapidus convinces Keamy to head back to the freighter with him. In the middle of the night, Claire is approached by Christian, and walks off with him, abandoning Aaron.

In flashforwards, Jack and Kate are a couple, and are raising Aaron together. Jack visits Hurley at Santa Rosa, who tells him that Charlie's ghost doesn't want him raising Aaron. Jack proposes to Kate, who accepts. However, their relationship falls apart as Jack is haunted by visions of Christian, and becomes jealous when he finds out that Kate is doing a favor for Sawyer and she refuses to tell him what it is.


Tropes in this episode include:

  • Actually Pretty Funny: Charlotte can't help but grin when she overhears Sun and Jin talking about Daniel's crush on her.
  • Big Brother Instinct: Sawyer now starts demonstrating this towards Claire, telling Miles not to look at or talk to her when he notices him eyeing her up.
    Miles: What are you, her big brother?
    Sawyer: No. I'm the guy who's gonna put a boot in your face unless you say, "Yeah, I getcha."
    Miles: Yeah, I getcha.
  • Call-Back: Jin figures out that Charlotte speaks Korean when he notices her smiling at a joke Sun made about her, the same way that Kate figured out Sun spoke English in "Hearts and Minds".
  • Chronic Hero Syndrome: Exaggerated. Jack can't allow anyone else to be in charge of saving people, even when he is the person who needs to be saved, and tries to direct his own appendectomy. Juliet wisely decides to knock him out and finish the job herself.
  • Continuity Nod: At the Staff Station, Sun takes a moment to look at the ultrasound table and tells Jin that this is where she saw the scan of their baby back in "D.O.C."
  • I Want My Beloved to Be Happy: Juliet respectfully bows out of the race for Jack's affections after realizing that he truly loves Kate.
  • Metaphorically True: After the events of "There's No Place Like Home, Part 2", it becomes clear that Jack's claim that Sawyer "chose to stay" on the Island is this. In reality, Sawyer jumped off the helicopter as it was headed for the freighter because it was leaking fuel, in order to ensure that the others made it.
  • Shipper on Deck: Sun and Jin gossip about Daniel's obvious crush on Charlotte, and seem to like the idea of them getting together.
    Jin: Do you think she knows he likes her?
    Sun: She's a woman. She knows.
