- Derek and Meredith are trying to film Zola's first steps, but she's not ready to "perform." Meanwhile, the nurses are setting up what Voiceover Meredith is now referring to as the "operating theater." It's all leading to a big moment. The surgeons are split into two teams. Hunt is preparing them for a big surgery -- they're going to separate conjoined twins. But first, they have to practice on dolls.
Dr. Warren wants Bailey to move in with him after they've been together for "two weeks and nine months," according to him. She's resisting.
The conjoined twins will be an 18-hour surgery, and Arizona is harping on every detail with each surgeon involved. Webber warns Alex not to mess it up.
Lexie is outside explaining the process to the twins' parents -- a young couple.
Bailey pulls Meredith from the practice session to tell her about her personal dilemma. Bailey is loathe to tell Meredith about her personal life, telling Meredith it's none of her business and, "the fact that I know things about your personal life is one of the great sadnesses of my existence."
"Sorry about that," Meredith says.
Bailey continues, telling Meredith she's personally involved with Dr. Warren, which Meredith matter-of-factly says "everyone knows." Bailey asks Meredith to pretend she doesn't know. Meredith feigns surprise. Bailey wants Meredith be a buffer between Bailey and Warren during Bailey's surgery -- pulling Meredith out of the "once-in-a-lifetime" conjoined twins procedure.
Cristina also leaves the practice after getting a message from Teddy. Arizona is still anxious about the details of the surgery on the twins, which she has treated since they were born. Hunt finally cuts off all the practice, and rolls the twins into the O.R., announcing, "It's showtime!"
In the twins' surgery, Arizona is annoyed Avery and Sloan seem to be racing to finish their procedures. They deny they're racing, but then each announce they think they've finished first -- then race to the cafeteria.
In Bailey's surgery, Warren talks Meredith up about how Zola is in the hospital's day care, which means it must be nice to have the whole family in one building. Meredith realizes she's been pulled into the personal conversation. She and Bailey go through the mundane details of their surgery to divert the conversation.
Cristina arrives in Teddy's O.R., where Teddy asks her to retell her all the details -- step by step -- of Henry's surgery and the complications, and the steps she and Dr. Webber took to address them, all the way until she called Henry's time of death. Teddy listens while operating on a patient, much to Kepner's visible shock. Cristina finishes the recounting of the ordeal. Teddy takes it in for a moment, not saying a word before asking Cristina to repeat it, which she does.
Back in Bailey's surgery, Bailey has suddenly launched into how Derek and Meredith's living together has ruined their lives -- up to and including Meredith's tampering with the Alzheimer's trial. Meredith maintains her life is pretty great. Warren settles the whole dispute by noting it "must be nice" the last person Meredith sees every night is Derek.
Lexie's talking to the parents. The young father says he has to get away and asks if there's a bar nearby. He leaves. The mother gets defensive, saying everybody thinks she accidentally got pregnant. She says she and her husband want their babies and will do whatever they have to do to make sure their babies are OK, "even if I have to sit around with judgy doctors."
In the twins' surgery, Alex is nervous about doing the flip of the twins -- which is a tricky and critical point in the process. Alex tells Hunt that Arizona doesn't trust anyone and is putting everyone on edge. Hunt asks who wants to replace Alex on the flip. Webber steps in.
Alex is upset watching Webber do the flip, and taking a round of applause after it's done successfully.
Back in Teddy's O.R., Teddy is still grilling Cristina on details about Henry's surgery. She questions whether Cristina was sure about the extent of Henry's tumor. Cristina assures her she was sure and wouldn't have called it without being sure. Teddy asks Cristina to start over.
Meredith leaves. Warren takes the moment to push Bailey a little more, saying they're "not kids anymore". He wants to share his life with her. He says she doesn't want the same things. They just might not be right for each other. Bailey says "it's been two weeks." He leaves, saying he got his answer.
Some of the surgeons eat lunch in the gallery while watching the twins' surgery. Meredith has Zola. Alex is still mad at Webber for taking his spot in the flip. Kepner pipes up, saying that what Teddy is doing to Cristina is "mean,". Cristina insists it's fine and she can handle it.
Derek and Callie run into a cyst and have trouble figuring out how to approach it. Arizona is providing more commentary than Callie would like, and tells her that since she and Derek are the ones operating on this part, they will make the decision. Derek then asks for "complete silence."
Lexie is sitting with the young mother when the dad comes back with two baby outfits he bought in the gift shop. They kiss and say they love each other, then start seriously making out -- much to Lexie's surprise. She tries to tell them the twins are about to separated. They're too busy sucking face to notice.
Derek and Callie complete the separation of the twins. There's some applause. Derek wants to check motor functions on both of them first. The first baby responds immediately to stimulation, wiggling her tiny foot. But the other doesn't. Derek asks for the stimulation to be cranked up a bit. After some tense moments, the second baby's foot wiggles.
Sloan is working on one of the twins and tells Hunt he "nailed it" with his procedure. Hunt goes to check on Avery's progress. Avery hadn't begun yet because Arizona and Alex are trying to repair the baby's kidney. In the waiting room, Lexie explains the complication to the young parents. The mother is upset with the doctors for having convinced them they could do the procedure. She says her daughter started the sharing three kidneys and now might not even have one. Lexie says she doesn't know if it will be OK.
Teddy wheels a gunshot victim into her O.R., where Cristina is standing off to the side. Kepner asks Cristina if she's going to scrub in this time. Cristina and Teddy both say "no" at the same time. Teddy scrubs in, then asks Cristina to pick up where she last left off on the recounting of Henry's surgery. Kepner looks upset.
Warren tells Bailey she doesn't have room in her life for "joy." Bailey responds by saying she likes to stay focused at work, which is why she doesn't make any mistakes. Meredith tells Bailey they're missing a sponge. Bailey keeps talking about how focused she is, until the nurse also confirms they're missing a sponge. They've looked everywhere -- except inside the patient.
Arizona tells Webber she's out of options. Alex suggests a kidney transplant. The baby's twin is the perfect match -- and they used to share three kidneys. Arizona tells Alex to "make it happen."
The young mother has just gotten a look at the first of her twins when Alex comes to take the healthy baby back into the O.R. He quickly explains the transplant idea. The young mother begins sobbing.
Hunt removes the kidney and hands it to Alex, who brings it over to the other O.R. As Alex goes to scrub in, Webber looks toward with a look of approval.
Teddy is pressing Cristina once again on all the details until Kepner finally boils over and shouts, "Enough! This is enough! You can't keep doing this. He's gone." Kepner tells Teddy, "This is not going to bring him back, so please." Teddy gives Kepner a stern look and asks if she's alright or needs time to gather herself. Kepner says she'd fine and puts her head back down. Cristina goes on with the recounting of the story.
Webber apologizes to Alex for taking away his moment. He says it was "sad" and "pathetic" -- "an old man acting out of fear." He explains the realization that he has "more yesterdays than tomorrows." He says he felt he had to fight like a tiger to keep himself in the game. Alex tells Webber he doesn't have to worry about that. Webber says a year or two from now he'll be stuck in an office in a part of the hospital where nobody goes, and be appointed to some board that makes up rules nobody pays attention to, and they'll let him "do a couple appies a week" so he can still call himself a surgeon. "You reach a certain age, that's what happens," Webber says. "That's the order of things. I can't think of anything more humiliating. Can you?"
"No," Alex says, before offering to let Webber scrub in on the transplant. Webber says Arizona only needs one of them. Alex insists Webber scrub in. Webber thanks him.
Bailey and the nurses are still having trouble finding the missing sponge. Meredith says they need to close the patient. Bailey insists she check under the liver again. Meredith says the sponge isn't in the patient, because it's stuck under Bailey's shoe. Warren mocks Bailey for being "focused, like a laser."
Back in the kidney transplant, Arizona asks Webber why he's doing it and not Alex. Webber says he "picked Karev's pocket." Then goes on to tease Alex for having done this twice in the same day. He says "it was like taking candy from a baby," then corrects himself because "that's not fair to the baby." Alex watches on as the transplant is complete. Everyone cheers again.
Cristina is telling Teddy again about Henry's surgery.This time, Teddy tearfully finishes the story for her. And Teddy cries some more. Cristina says, "I'm sorry." Teddy tells her not to apologize, saying she did everything Teddy would have done.
"You were me in that O.R.," Teddy says. "You were good, and you did it right. He just died." Cristina and Kepner's eyes both fill with tears as they watch Teddy silently.
Bailey tells Warren she was married and had it all, and it fell apart. Then she found Warren, and there was a shooting. She doesn't want things to blow up again. He tells her they can take it slow, and she reminds him about her son. He says he loves Tucker and wants the whole package. She agrees to take it "one step at a time."
Arizona is giddy after the surgery. Callie tells her she was a monster.
Alex gives Webber a mini-speech about how "this is a teaching hospital" and he should be teaching the surgeons rather than stealing surgeries from them. Webber tells Alex he taught him something: Not to give up a once-in-a-lifetime surgery to anyone for any reason. He then tells Alex to watch his back, because he might decide to teach him something tomorrow.
There's a quick misunderstanding, when Sloan says, "God, I wish you could've been in there with me today," as Lexie is walking by. Lexie says, "Really? Me, too!". She then sees Sloan was talking into the bluetooth device he had in his other ear. She walks away, flustered.
The parents get to meet their newly separated twins, and that night Derek and Meredith are exhausted at home. They watch the video of Zola from that morning, where she was close to walking but didn't do it, and Derek says she looks like she's bigger already. Suddenly, Zola takes her first steps. Derek fumbles for the video camera and can't turn it on in time to catch the big moment.
