- The latest number the machine dispenses belongs to heart surgeon, Dr. Madeleine Enright, known as the best in her field. That is why wealthy businessman Oliver Veldt chooses her for his heart surgery, which is kept secret from the public and for which the entire wing of the hospital is to be kept secure for his safety. Finch and Reese are certain that she is the intended target. But they quickly learn that she is the unwilling perpetrator. Just before the operation is scheduled to begin, she is threatened by a mysterious Brit named Alistair Wesley to kill Veldt on the operating room table and make it look like an accident, or else Wesley will get one of his many operatives to kill Maddy's wife, Amy, who, during the duration of the operation, is left vulnerable in a wide open and busy public space, where the many unknown operatives can monitor her movements. Reese and Finch also learn that Wesley has at least one operative somewhere in the hospital monitoring what Maddy is doing in the operating room. With the help of one of their old persons of interest who again is in trouble due to his own doing, Reese and Finch have to find out for whom Wesley is working to discover why he is intent of killing Veldt before Maddy has to make the decision to sacrifice Veldt or possibly her wife Amy. Meanwhile, Fusco calls in Carter on a homicide that seems to have a personal connection to her.—Huggo
- The Machine delivers the number of Leon Tao again, and Reese saves him from two thugs and brings Leon blindfold to Finch's hideout. When he arrives, Finch gives the number of the heart surgeon Dr. Madeleine Enright, who is considered to be the best in her field. They leave Bear watching Leon, while Finch poses as investor of the hospital and Reese sees Dr. Enright meeting her wife Amy in the cafeteria of the hospital. Soon the powerful Oliver Veldt arrives to be submitted to a secret heart surgery, and the whole floor is isolated for his safety. However, Dr. Enright is approached by the mysterious British Alistair Wesley, who threatens the life of Amy that is in a nearby square. He tells her to kill Veldt accidentally; otherwise, his operatives will kill Amy. Meanwhile, Fusco is investigating a murder scene and finds Carter's card in the pocket of the victim. He warns Carter, but she does not know who the victim is.—Claudio Carvalho, Rio de Janeiro, Brazil
- Finch tells Reese that they have a "repeat number." At that exact moment Leon Sung (Ken Leung) is thrown through a window and lands at Reese's feet. Reese rescues him from two large members of the Russian mafia and brings him to Finch. They need to attend to other matters and tell him to stay there under the watchful gaze of their adopted Belgian Malinoi, "Bear" (Graubaer's Boker).
Finch brings Reese to a hospital. Finch has made a huge donation, allowing them to meet a preeminent surgeon named Madeleine "Maddie" Enright (Sharon Leal). Finch is able to replace Maddie's eyeglasses with a bugged pair. The men are asked to leave the wing because of a sensitive operation.
We see Maddie meet with Oliver Velt, a CEO of an energy company. He's having heart surgery that he needs to keep secret from shareholders.
Maddie and her wife Amy (Erica Leerhsen) have been married for a year. Reese overhears them talk about the big surgery that's coming up.
Meanwhile, while working an execution-style murder case Fusco finds Carter's card in the victim's pocket.
A man named Alistair Wesley (Julian Sands) waits for Maddie in the lounge. He says he has a gun pointed at Amy and she'll be killed if she doesn't ensure Velt "accidentally" dies during surgery. He gives her specific instructions as to how to kill him. If the police are contacted or Amy notified, her wife will die.
On the back of Carter's card someone had written "6611TH." Carter and Fusco aren't sure what it means.
Prior to surgery Maddie gets a text reminding her what she needs to do. She mixes a special solution in a hypodermic needle and hides it in her pocket.
Reese sneaks into Maddie's office while Reese goes to Amy's location and starts looking for the sniper. Finch is able to stall the surgery by creating a shortage of Velt's blood type. Reese tracks down the sniper's hideout and shoots him during the struggle. Wesley calls the sniper's phone and informs Reese that there are multiple snipers. He wants to meet for a drink or Amy dies.
Carter looks up the man who was killed. She finds he was a custodian and that one of his jobs corresponds with a location on 66th Street and 11th Avenue.
Reese has Fusco keep an eye on Amy while he meets Wesley for his drink.
Finch calls Leon and has him look up who might be targeting Velt.
Wesley tells Reese he has a number of people close enough to kill Amy at a moment's notice. Reese guesses the team are former British military, perhaps SIS. Wesley tells him to stay away. Reese fights with one of his men who has tried to garrote him and Wesley gets away during the struggle.
While Maddie scrubs in for surgery, Finch tells her that they are working on her behalf. He tells her they're working to save Amy before she'll be forced to kill Velt. Reese gives Finch an update on how professional Wesley's operation appears to be.
Velt's surgery begins.
Leon calls Finch with info that a short-sale investor stands to gain millions if Velt were to die on the table. Finch wonders if perhaps Wesley's team is the investors.
Maddie says she needs to take a short break. Finch gives her an update and she appears to be panicking. Maddie gets a call from Wesley, who knows she has stopped her surgery. He connects her with Amy to remind her what she has to lose.
Finch and Reese realize Wesley must have eyes inside the hospital.
Carter arrives at the address of the company printed on her business card. It's a tech company and shots have just been fired. Carter spots Mark Snow (Michael Kelly) leaving the front and follows him.
Fusco creates a distraction with an officer near Amy's location.
Leon calls Finch back with more information. Apparently someone on the inside leaked information about the surgery to the short-sellers.
Reese calls Amy on a new phone and explains who he is and that he's going to direct her to safety. Finch calls him and says they have less than two minutes.
Carter catches up to Snow. He tells her she needs to tell "our friend" that something big in on the way and show her the bomb he's wearing. Shots are fired at them by an unknown party.
Finch sits next to Velt's assistant Rains and explains to him he knows that he's working for Wesley. He has enough information to send Rains to jail. Rains seems genuinely in the dark about what's he's talking about. Finch goes into the surgery room and tells Maddie they haven't been able to get Amy to safety. She calls off the surgery, declaring herself unfit. Finch orders Reese to get Amy out.
Wesley orders all of his associates to take Amy out.
Maddie begins making arrangements to get the instruments out of Velt's chest.
Reese seems to get Amy to safety until a man (Nick Stevenson) posing as a uniformed police officer pulls his gun. At the same moment Maddie sees that one of the nurses is on Wesley's payroll and has nicked one of Velt's arteries. Fusco distracts the fake cop long enough for Reese to get the jump. And Maddie takes over and begins to save Velt. The renegade nurse pulls a scalpel on Maddie but Finch manages to knock the nurse unconscious. Finch ends up having to help Maddie get Velt's heart started.
Maddie and Amy are reunited. Leon is sent on his way.
Reese gets a call on the sniper's phone. It is Wesley congratulating him. They have a brief conversation before Wesley hangs up and discards his cell phone in a trash bin as he heads for a flight out of the country.
Carter tells Reese about the conversation with Snow. The message is that "she is planning something. Something big." Reese asks her how much more she really wants to know.
