- A respected doctor is poisoned with Polonium and has just 24 hours to help Reese find his killer. Carter, grieving over the death of Cal Beecher, helps Fusco when HR turns Internal Affairs against him.
- Reese first catches up with the latest number, Dr. Richard Nelson, when he is being awarded the title of professor emeritus at a lavish dinner banquet. Dr. Nelson is a well respected and renowned cardiac surgeon and researcher, with his credentials suggesting that he is the target. That is indeed the case, but Reese and Finch are too late: someone has poisoned Dr. Nelson after he ingested something at that banquet that was slipped into his food or drink. Dr. Nelson first believes that Reese is the culprit, but Reese convinces him that he is only there to help find who did it, even if Reese cannot save him as he finds out the poison is radioactive, which is eating Dr. Nelson's insides and which also means that he will probably die within 24 hours. Reese, Finch and Dr. Nelson have to find out what Dr. Nelson ate or drank that was the source of the poison, and wade through the many facets of his life to find the killer. Among the suspects are Dr. Nelson's estranged daughter Molly, or someone tied to the drug trial - for which Dr. Nelson sat on the review committee - which denied that drug's approval at the eleventh hour. Meanwhile, Simmons threatens Fusco to keep on playing ball with HR or else. Simmons is good on his threat as internal affairs does come knocking on his door regarding the disappearance of Detective Stills. Although he did not kill Stills, he did bury the body. Carter didn't want to hear about Fusco's shady past before, but she now has to determine if she will help him or let Internal Affairs find what Simmons wants them to find.—Huggo
- The Machine delivers the number of the prominent cardiac surgeon Dr. Richard Nelson and Reese poses as waiter in the party where he is awarded with the title of professor emeritus. However, Dr. Nelson does not feel good and Reese concludes that he arrived late and the surgeon was poisoned. Finch investigates and they conclude Dr. Nelson was poisoned by Polonium and has only twenty-four hours to live. Reese teams-up with Nelson to find who might have done such cruelty. Meanwhile, Simmons has put Internal Affairs to chase Fusco, and he recalls his mistakes in the past. They are looking for the body of Det. Stills that Fusco did not kill but buried his body. Carter asks Reese whether he will help Fusco, but Reese is not available. So Carter decides to help him in an unexpected way.—Claudio Carvalho, Rio de Janeiro, Brazil
- Open in 2011 with Fusco dragging a body in the snow. We hear a phone conversation where somebody says they know what happened to Detective Stills, and so does Fusco.
We see the award ceremony for the newest number, a heart surgeon named Dr. Richard Nelson. Finch thinks it's unlikely Nelson is a perpetrator. Nelson notices a rough-looking young woman in the back of the room and pauses.
We see Quinn speak at Cal's funeral.
After Nelson's speech he's approached by a patient, a trader at a high-profile hedge fund named Boyd. Nelson's next approached by a doctor named Garrett who works for a pharmaceutical company. They discuss a failed drug trial but it seems to be friendly. Finch mentions to Reese that Nelson's role in the failed trial cost Garrett's company billions.
After the funeral Fusco approaches Simmons and says he's going to take him down for his role in Cal's murder. Simmons responds that he's coming after his career for past transgressions
Next Nelson speaks with a former protégé named Juliana. They reference his daughter Molly. Nelson's nose begins to bleed and his tells Reese (who is posing as a server) that he feels lightheaded.
A detective named Soriano from IA takes Fusco's badge and gun because of a possible connection to Stills. Carter pops into the interrogation room and asks Fusco what's going on. Fusco says some chickens have come home to roost and she should stay away from him.
Finch calls Carter looking for some help with Nelson. She says no, instead wanting help with phone cloning.
Finch listens to a phone call between Nelson and Molly. It seems there are issues there. Nelson's nose continues to bleed and he vomits blood. Nelson calls 911 and says someone tried to kill him. He wanders into the street and runs into the police. Reese tells Finch he thinks they're too late to save Nelson.
Soriano asks Fusco about working with Stills, who he worked with coming up in the department. We flash back to 2004. Fusco is cut off at a bar and bonds with Stills about the breakup of his marriage. Stills invites him to stay at his place.
Back in present day Finch finds traces of a polonium in Nelson's lab. For it to be lethal it must be ingested.
Reese pays Nelson a visit in the hospital. He tells him about the polonium and says in less than 24 hours he'll be dead. He asks him to come with him and find the person who murdered him.
Nelson goes to see Molly. He asks her how she's doing and apologizes for his parenting, but doesn't mention that he's dying.
Carter listens in on the interrogation of Fusco. A dirty cop named Azarello confessed to Fusco and Stills helping them out. This leads to a flashback of Fusco helping to cover up a murder.
Carter pays Azarello a visit inside. He tells her he's finally talking because Fusco violated a code. She asks him if he wants to help her out, but he says she's too late.
Carter tells Finch that Boyd is being investigated by the SEC, something related to insider trading and a drug trial.
Reese tells Nelson that Boyd made $500 million because of information the doctor gave him about a failed drug. Nelson was the liability, the final link to the insider trading. Nelson wants to talk to Boyd.
Finch thinks the origin of the poisoning was the water on Nelson's podium.
Reese and Nelson grab Boyd off the street. Boyd is adamant he had nothing to do with the poisoning. He blames his boss Vincent for pressuring him to get insider info. He tells them where they can find Vincent.
We get another flashback to Fusco arriving at a drug scene with a dead body. Fusco doesn't want to share in any of the dirty money/drugs, but ends up saving Stills and Azarello's life when a scumbag pops out from a closet.
Soriano shows Fusco he has evidence that he covered up evidence the day Stills went missing. He has cell phone records that Fusco was in Oyster Bay that day. There is also satellite photos of four locations where soil has been disturbed. In the morning Soriano is going to send officers to dig up the locations.
Carter talks to Fusco. He tells her didn't kill Stills, but he's going to be busted for it. Fusco thinks he deserves it because of his sketchy history. He tells her that he killed people but assures her he's no longer the same guy.
Reese has to inject Nelson with adrenaline to bring him back from near-death. He tells him he can't die until they finish one more mission.
Nelson sits with Vincent in his upstate lodge.
Carter calls Reese looking for help with Fusco. He tells her that he's busy and she'll have to take care of the situation on her own.
Nelson talks to Vincent about the way he's dying. Vincent doesn't seem to care all that much and tells Nelson "I always win." Nelson reveals that he's poisoned Vincent and he now has 24 hours to live.
Fusco is taken to the locations where Stills is supposedly buried.
Nelson talks to Molly on the phone one more time. He and Reese enjoy a glass of his favorite scotch. Nelson thanks Reese for allowing him one more shot, then he dies. Rese and Finch talk about the fact they're starting to arrive too late to save people.
The police find that Stills' body has recently been removed. Fusco is off the hook, at least for the moment.
Finch tells Reese he thinks trouble is coming. The machine has been infected with a virus.
We see that it was Carter who dug up and moved Stills' body.
